Gonstead Technique of Adjustments

The Gonstead system of chiropractic involves a specific form of analysis that utilizes a patient and region-centered, weighted, multiparameter examination consisting of patient diagnoses, symptoms, and physical findings, posture analysis and inspection, motion and static palpation, neutral and stress position xrays, and skin temperature instrumentation to arrive at the location and nature of a joint subluxation or sprain. A subluxation is defined (Oxford's Dictionary of the English Language) as a partial dislocation, a sprain.


The adjustment is intended to be delivered in such a way that joint approximation occurs, that fixation or hypomobility is reduced and any associated neurological dysfunction is normalized. When an adjustment is performed usually a joint cavitation or audible sound occurs.

Every joint sprain is different in every patient. A joint sprain at the ankle is different from a lower lumbar joint sprain. A neck sprain in an infant who has fallen from a changing table  on to their head is different from a joint sprain that is 50 years old and has progressed into degenerative joint disease/osteoarthritis in an older adult.

Adjustment are made using different tables and position from other types of adjustment approaches. Chiropractors using the Gonstead method attempt to adjust joints in neutral positions and tend to use specific contacts on bones and the avoidance of long lever techniques.

An integral part of the Gonstead system is the use of adjuncts to care such as nutritional and dietary advice, and rehabilitative exercises.

The Gonstead system is evidence-informed. What follows are some specific citations of evidence related to various elements of the Gonstead system in terms of joint problems and some conditions.

After these few highlights, what follows is a complete Gonstead bibliography. Please consult the full papers for complete information. Of interest to a patient may be the quite substantial list of case reports involving many different kinds of diseases. Case reports are not randomized clinical trials or Class A evidence. Some types of conditions only lend themselves to investigation through a case report or case-series approach. Your personal results will vary. That a report exists that something happened to somebody, once, and was published, doesn't mean your result will be the same.

Leg Pain

This is a study of Gonstead technique in patients with disc problems and sciatica:
Chiropractic manipulation in the treatment of acute back pain and sciatica with disc protrusion: a randomized double-blind clinical trial of active and simulated spinal manipulations.

METHODS:

Manipulations or simulated manipulations were done 5 days per week by experienced chiropractors, with a number of sessions which depended on pain relief or up to a maximum of 20, using a rapid thrust technique. Patients were assessed at admission and at 15, 30, 45, 90, and 180 days. At each visit, all indicators of pain relief were used.

RESULTS:

A total of 64 men and 38 women aged 19-63 years were randomized to manipulations (53) or simulated manipulations (49). Manipulations appeared more effective on the basis of the percentage of pain-free cases (local pain 28 vs. 6%; p<.005; radiating pain 55 vs. 20%; p<.0001), number of days with pain (23.6 vs. 27.4; p<.005), and number of days with moderate or severe pain (13.9 vs. 17.9; p<.05). Patients receiving manipulations had lower mean VAS1 (p<.0001) and VAS2 scores (p<.001). A significant interaction was found between therapeutic arm and time. There were no significant differences in quality of life and psychosocial scores. There were only two treatment failures (manipulation 1; simulated manipulation 1) and no adverse events.

CONCLUSIONS:

Active manipulations have more effect than simulated manipulations on pain relief for acute back pain and sciatica with disc protrusion.

This is a study of Gonstead adjustments. Of note is a common joint misalignment (retrolisthesis) of the lower lumbar spine and the effects of these adjustments

The examination included history, vital signs, review of systems, an orthopedic examination, a brief neurological screen, and a spinal examination. Patients were excluded if contraindications to manipulation were found. Two licensed chiropractors evaluated the spinal segments for hypomobility using palpatory examination and delivered HVLA-SM to the lumbar spine of the participants in a side-lying posture. The participant was positioned in a side-lying (lateral recumbent) posture. The superior or free hip and knee were flexed and adducted across the midline. The chiropractor stabilized the participant by leaning his/her own leg against the participant’s superior leg, while holding the participant’s superior shoulder. The manipulation was applied with the chiropractor’s hand by thrusting into the spine or sacroiliac joint. The thrusting load is a quick, short, controlled movement of the shoulder, arm and hand combined with a slight body drop...... The two chiropractors delivered a total of 14 thrusts to the five asymptomatic volunteers. Table 1 summarizes descriptively the 3-D forces, durations, and loading rates for all 14 spinal manipulations as well as for the two chiropractors. Normal force (Fz) is the dominating force, followed by inferior-superior force (Fx), and the lateral force (Fy) which occurred in both directions....''
