The First Visit and Fees

The initial consultation is complementary and provides us an opportunity to see if my services are a right fit for your health care needs.

When someone contacts a doctor, their concern or reason for the contact is called the chief complaint. However, the chief complaint is but the door to the house, and not the house itself. My approach is to address the chief complaint but also to examine thoroughly to determine other problems and causes that are relevant and can be addressed by chiropractic methods or let me know that I need to refer you to another type of doctor or a hospital. My back hurts (lumbago) is not a good diagnosis. A good diagnosis is one that also addresses the cause of the symptom. I am interesting in finding causes of problems and addressing the causes with natural methods without the use of drugs or surgery.

A patient may have a back spasm. But why is the muscle in spasm? Is it because there is a muscle strain from over exertion?
Is it because there is a bulging disk compressing a nerve and the body is innately protecting the nerve from further injury by splinting the area and making it more difficult to move?

Or is the back in spasm because of a kidney stone or bowel problems?

A headache could be from a displaced vertebra or abnormal posture of the neck but a headache can also occur because of high blood pressure.

Pain can be thought of as a check engine light in a car. It is a signal for something else that needs to be diagnosed.

During the consultation, I will ask questions about your joints, muscles, and nerves but I will  also ask about your stress levels, whether you are anxious or depressed, if you take medications, how much you drink or smoke, what your diet is like, how much you get in the sun and how much sleep you get, and whether and how you exercise.

The history also  involves me asking questions about traumas, falls, injuries, diseases you have had in the past, and if you have had any surgeries or hospitalizations.

A detailed history takes a few minutes. Then we move to physical examination where vital signs are obtained such as height, weight, blood pressure, breathing rate, heart rate and temperature.

After vital signs I examine your spine and other joints thoroughly and other relevant areas depending on what is uncovered during the history.

I will review any xrays/imaging studies or reports you have prior to an adjustment.

Every patient in our clinic has different needs and gets different services depending on those needs. It is difficult to determine your needs over the phone, before a consultation or examination has been performed. The clinic’s fees are sliding scale (based on financial need) and generally affordable. We provide discounts for prompt payment. We follow the California regulations (see 657 below) regarding fees for those who have financial need, lack coverage, or because the coverage they do have will not pay or will not pay promptly, or the plan has a high deductible or copay.

Because of severe financial hardship in some cases we provide care pro-bono/at no charge.

X-rays are extra and range from $30 to $200 dollars, depending on the number of films and regions/areas. If you need laboratory tests we participate in a professional co-op and obtain tests through Lab Corp at a lower cost. Laboratory costs are $40.00 or more depending on which tests are ordered. We also may refer you for MRI’s or special studies at centers that provide discounts for prompt payment. A basic MRI could cost $500.00.

The first visit for new patients consists of a general health history, physical examination, reviews of laboratory tests, imaging, X-rays, or special studies, a diagnosis, differential diagnosis or clinical impression, report of findings, an adjustment, and recommendations. The Gonstead Clinic has a comprehensive approach to your health care needs and the first visit can take 45 minutes to an hour or more depending on the complexity of the case. Follow-up adjustment-only visits are brief and only take less than ten minutes. In the case of rehabilitation such as balance exercises, visits can be 15-20 minutes or more.

California Business and Professions Code Section 657

(a) The Legislature finds and declares all of the following:

(1) Californians spend more than one hundred billion dollars ($100,000,000,000) annually on health care.

(2) In 1994, an estimated 6.6 million of California's 32 million residents did not have any health insurance and were ineligible for Medi-Cal.

(3) Many of California's uninsured cannot afford basic, preventative health care resulting in these residents relying on emergency rooms for urgent health care, thus driving up health care costs.

(4) Health care should be affordable and accessible to all Californians.

(5) The public interest dictates that uninsured Californians have access to basic, preventative health care at affordable prices.

(b) To encourage the prompt payment of health or medical care claims, health care providers are hereby expressly authorized to grant discounts in health or medical care claims when payment is made promptly within time limits prescribed by the health care providers or institutions rendering the service or treatment.

(c) Notwithstanding any provision in any health care service plan contract or insurance contract to the contrary, health care providers are hereby expressly authorized to grant discounts for health or medical care provided to any patient the health care provider has reasonable cause to believe is not eligible for, or is not entitled to, insurance reimbursement, coverage under the Medi-Cal program, or coverage by a health care service plan for the health or medical care provided.  Any discounted fee granted pursuant to this section shall not be deemed to be the health care provider's usual, customary, or reasonable fee for any other purposes, including, but not limited to, any health care service plan contract or insurance contract.

(d) “Health care provider,” as used in this section, means any person licensed or certified pursuant to Division 2 (commencing with Section 500) of the Business and Professions Code, or licensed pursuant to the Osteopathic Initiative Act, or the Chiropractic Initiative Act, or licensed pursuant to Chapter 2.5 (commencing with Section 1440) of Division 2 of the Health and Safety Code;  and any clinic, health dispensary, or health facility, licensed pursuant to Division 2 (commencing with Section 1200) of the Health and Safety Cod
